Our service includes robust medicines management, close monitoring of physical and mental health needs, and proactive liaison with GPs, psychiatrists, psychologists, and other professionals. We work closely with families, advocates, and commissioning teams to ensure care remains responsive and outcomes-focused.
Where required, we provide dedicated 1:1 support to ensure individuals are safe, well-supported, and able to access meaningful activities both within the home and in the community.
